Lower back pain is an extremely common condition with up to 80% of the population experiencing back pain at some time during the lives. Although the underlying pathology of low back pain is usually not serious, it is an important cause of pain in the community and has considerable impact on the economy due to time taken off work. Whilst the vast majority of patients with back pain will recover within 6 weeks, a small number will go on to develop  chronic back pain and a gradual deterioration of functionality. 

Causes of Back Pain

Back pain is frequently caused by irritation or inadequate function of the bones, joints, ligaments, or muscles in your back. Vertebral dysfunction, a common condition that arises when the spinal vertebrae compress or irritate the delicate nerve structures in your lower back, may be the culprit behind such irritation. 

Inflammatory arthritis (e.g., ankylosing spondylitis, rheumatoid arthritis), as well as fractures caused by osteoporosis, are other common causes of lower back pain. Additionally, back pain may be “referred” from another location. 

Low back pain caused by cancer (usually originating from another source such as the prostate or lung), severe infections, and compression of the spinal cord are less frequent occurrences.

Assessment of Back Pain

To identify the precise cause of your lower back pain, your physiotherapist will conduct a comprehensive examination of your history, orthopaedic, neurological, and spinal functions. Additional evaluations, such as x-rays and digital scans, may also be advised. 

Treatment for Back Pain

In the treatment of low back pain, physiotherapy focuses on restoring function to spinal joints that are moving poorly, as well as reducing muscle tension and increasing muscle strength. This results in improved movement, reduced pain, and decreased nerve irritation. 

The treatments are customised to the individual case and generally consist of spinal and soft tissue mobilisation, advice on core stabilisation, strengthening exercises, stretching, and heat/cold therapy.
